Job Description:

Nurse Case Manager responsible for assessing, planning, implementing and coordinating all case management activities with members to evaluate medical needs and facilitate overall wellness. Position involves local travel (50-75%) within Dallas, Denton, Fort Worth, Hunt, Collin and/or Tarrant county.

Job Responsibility:

Telephonically and/or face to face assessing, planning, implementing and coordinating all case management activities
Develops proactive course of action to address issues
Coordinates and implements assigned care plan activities
Conducts multidisciplinary review
Utilizes negotiation skills to secure appropriate options and services
Provides coaching, information and support to empower members
Monitoring, Evaluation and Documentation of Care

Requirements:

License Registered Nurse
3+ years clinical practice experience
1+ year Case Management experience
Bachelors degree preferred

Nice to have:

MCO experience
Star Kids experience
Bilingual preferred
