Job Description

The Case Manager RN role is a Work at Home/Hybrid role based on your location. Hybrid - if you are located within 30 Miles of 4050 Piedmont Parkway High Point NC. Normal Working Hours: Monday through Friday 9:00am – 5:30pm EST. Occasional evening, weekend, and holiday shifts per the needs of the team. Possible travel up to 10% of the time to High Point NC.

Requirements

  • Oncology Experience or certification
  • Active and unrestricted RN license in the state of North Carolina
  • Willing and able to obtain multiple state RN licensure after hire (expenses paid for by company)
  • 2 or more years of clinical experience as an RN
  • Associates Degree in Nursing

Nice to have

  • Certified Case Manager (CCM) certification
  • 3+ years' experience with Microsoft Office Suite
  • Case Management in an integrated model
  • Discharge Planning experience
  • Managed care experience

  • Responsible for continuous review of all aspects of every patient of her/his caseload to include appropriate utilization of services and the appropriate hospice level of care criteria are met
  • Makes referrals to other team members as appropriate
  • Administers medication and treatments as prescribed by the physician
  • Leads the interdisciplinary care group (IDG) specific to each patient and family on their caseload
  • Working within the IDG to develop, revise, and implement the patient’s Plan of Care
  • Coordinates all services and collaborates with members of the interdisciplinary team to provide consistent patient care
