Job Description:

This position is accountable for assessing, planning, implementing, coordinating, monitoring, and evaluating options and services available to meet a patient's health needs. Coordinates services provided to a specified group of patients, ensuring that the care is provided in an appropriate setting, and that the severity of illness and the intensity of service is consistent with the patient's clinical condition. Collaborates with the health care team members to ensure that the care provided meets desired patient outcomes with appropriate utilization of resources within expected time-frames across the continuum of care. This position practices within the scope of the Nursing Practice Act, and in accordance with the policies on Valley Children's Healthcare.

Nice to have:

  • College Degree - 4 year Nursing or in a field that supports the Case Management's process
  • Pediatric experience and Case Management/Utilization Management experience
  • Bilingual Skills
