Job Description:

This position is accountable for assessing, planning, implementing, coordinating, monitoring, and evaluating options and services available to meet a patient's health needs. Coordinates services provided to a specified group of patients, ensuring that the care is provided in an appropriate setting, and that the severity of illness and the intensity of service is consistent with the patient's clinical condition. Collaborates with the health care team members to ensure that the care provided meets desired patient outcomes with appropriate utilization of resources within expected time-frames across the continuum of care. This position practices within the scope of the Nursing Practice Act, and in accordance with the policies on Valley Children's Healthcare.

Job Responsibility:

Requirements:

  • College Degree - 2 year Nursing (required)
  • RN - Registered Nurse License - CA-BRN - California Board of Registered Nursing (required)
  • Minimum two (2) years Nursing experience (required)
  • College Degree - 4 year Nursing or in a field that supports the Case Management's process (preferred)
  • Pediatric experience and Case Management/Utilization Management experience desirable. (preferred)
  • Bilingual Skills Preferred.

  • Must maintain clinical privileges at the hospitals of the attending plastic surgeons within the group

  • Assists in the preoperative assessment and preparation of the necessary paperwork and orders for scheduled cases
