Job Description:

Exciting opportunities exists for two suitably experienced and qualified individuals to take on case management roles within the Hume Merri-bek team. The suitable candidates will have knowledge and/or experience working with young people experiencing mental illness, neurodiversity, disability; and those who may be part of the LGBTQIA+ community. Case Managers are responsible for providing individually tailored outreach support to disengaged learners within the context of their families, whilst developing and maintaining relationships with stakeholders across a range of sectors. The Navigator Hume Merri-bek team is based in Brunswick. These are full time, 6-month contract positions. Attractive salary in the range of $88,090 – $94,789 pro rata, plus salary packaging benefits, together with ongoing professional and career development.
