This role focuses on providing compassionate and effective support to underserved populations, including those facing homelessness, mental health challenges, or substance abuse issues.
Job Responsibility:
Conduct client intakes, perform assessments, and create comprehensive case plans tailored to individual needs
Provide crisis intervention, counseling, and practical support to help clients resolve urgent issues related to housing, income, transportation, or health
Monitor client progress, advocate for their needs, and assist with administrative tasks such as form completion, translation, and correspondence
Maintain accurate and organized client records, both physical and digital, while ensuring data integrity in relevant systems
Support front desk operations, including handling inquiries and generating statistical reports to evaluate program outcomes
Address client concerns and provide guidance to staff and volunteers, fostering a collaborative and supportive environment
Represent the organization professionally in interactions with clients, external agencies, and community partners
Attend required trainings, meetings, and adapt to flexible schedules, including evenings, weekends, and holidays
Report incidents promptly and adhere to organizational policies and procedures
Recommend improvements to case management processes to enhance service delivery and effectiveness
Requirements:
Bachelor’s degree in Social Work, Psychology, or a related field
Master’s degree in Social Work preferred
Minimum of 2 years of experience in case management or similar roles
Familiarity with community services and resources for economically disadvantaged populations
Experience supporting individuals facing co-occurring challenges such as mental health issues, substance abuse, homelessness, or trauma
Proficiency in Microsoft Office and ability to learn systems like Clarity Database
Bilingual in English and Spanish or Cantonese/Mandarin is highly preferred
Strong analytical, communication, crisis management, and organizational skills
Reliable transportation and a clean driving record
