Registered healthcare professional (HCPC / NMC / SWE)
Occupational Therapist, Nurse, Physiotherapist, Speech & Language Therapist or Social Worker
Minimum 6 years post-qualified experience
Strong community rehabilitation background
Experience with complex or catastrophic injury caseloads
Ability to work independently and within MDT structures

Job Responsibility

Managing complex rehabilitation caseloads across community and home environments
Completing holistic needs assessments and detailed case reports
Developing and coordinating bespoke rehabilitation plans
MDT coordination across health, legal, and social care professionals
Risk assessment and risk management planning
Budget management and cost-effective service coordination
Advocacy for clients and families throughout rehabilitation journey
Ongoing review and optimisation of care packages
Liaison with solicitors, insurers, and healthcare providers
Supporting vocational, educational and community reintegration
