Job Description:

This Care Management Coordinator position is with Aetna’s Long Term Services & Supports (LTSS) team and is a field-based position out of North Chicago IL. The requirements is for candidates to travel 50-75% of the time to meet with members face to face. This position holds a full caseload to manage waiver members. This position requires in person quarterly visits with members. This position is critical to meet contractual requirements.

Job Responsibility:

  • Facilitate appropriate healthcare outcomes for waiver/LTSS members by providing care coordination, support and education for members through the use of care management tools and resources
  • Evaluation of Members
  • Enhancement of Medical Appropriateness and Quality of Care
  • Monitoring, Evaluation and Documentation of Care

Requirements:

  • Must reside in the Northern area of Chicago IL, Preferably in zip codes 60656, 60666, 60068, 60631, 60076, 60077, 60203, 60201, 60204, 60208, 60209, or 60016–60019
  • Willing and able to travel up to 50% of their time to meet with members face to face in Northern Cook County IL, and surrounding areas
  • Reliable Transportation required, eligible for mileage reimbursement as per company policy
  • Minimum 2 years of experience in behavioral health, social services or human services field
  • Minimum 2 years of case Management experience

Nice to have:

  • Case management and discharge planning experience
  • Managed Care experience
  • Microsoft Office experience
