Job Responsibility:
Coordination of "Important Message from Medicare" (IM) correspondence
Obtains signatures from Medicare Beneficiaries or their emergency contact within 48 hours prior to discharge
Mails out appropriate letters to emergency contacts
Monitors all department voicemail lines and follows up appropriately
Delivers the IM letter/Code 44 letter to the Medicare Beneficiary or their emergency contact within 48 hours prior to discharge
Documents all activities related to the IM/Code 44 in Care Connection
Provides clerical support to the Care Managers and Social Work Staff during the appeals process for a Medicare Beneficiary
Coordination of discharge planning
Provides required documents to vendors in a timely manner
Assists with facility bed availability and updates the Care Manager or Social Worker of status of facility as needed
Identifies contracted vendors for discharge relating to home health care, nursing home placements, durable medical equipment, LTACHs, Hospice etc.
Compilation of department statistics as requested
Updates hospital systems related to discharge
Provides reports as needed
Provide Clerical support to the department
Order supplies for the department
Take meeting minutes for departmental as well as ad hoc meetings when requested
Requirements:
H.S. diploma or equivalent
2-3 years clerical experience in a health care setting
Ability to use Microsoft Office
Ability to handle difficult calls and maintain professional conduct
Good communication skills
Prolonged sitting and data entry
Positive interpersonal skills
Effective communication skills
Able to work under deadlines and in fast-paced environment
Able to prioritize independently and respond to multiple, simultaneous requests
Able to maintain patient privacy and confidentiality
