Job Responsibility

  • Coordination of "Important Message from Medicare" (IM) correspondence
  • Obtains signatures from Medicare Beneficiaries or their emergency contact within 48 hours prior to discharge
  • Mails out appropriate letters to emergency contacts
  • Monitors all department voicemail lines and follows up appropriately
  • Delivers the IM letter/Code 44 letter to the Medicare Beneficiary or their emergency contact within 48 hours prior to discharge
  • Documents all activities related to the IM/Code 44 in Care Connection
  • Provides clerical support to the Care Managers and Social Work Staff during the appeals process for a Medicare Beneficiary
  • Coordination of discharge planning
  • Provides required documents to vendors in a timely manner
  • Assists with facility bed availability and updates the Care Manager or Social Worker of status of facility as needed
  • Identifies contracted vendors for discharge relating to home health care, nursing home placements, durable medical equipment, LTACHs, Hospice etc.
  • Compilation of department statistics as requested
  • Updates hospital systems related to discharge
  • Provides reports as needed
  • Provide Clerical support to the department
  • Order supplies for the department
  • Take meeting minutes for departmental as well as ad hoc meetings when requested

Requirements

  • H.S. diploma or equivalent
  • 2-3 years clerical experience in a health care setting
  • Ability to use Microsoft Office
  • Ability to handle difficult calls and maintain professional conduct
  • Good communication skills

  • Able to work under deadlines and in fast-paced environment
