Job Description:

We’re Hiring: Case Assessor – Adult Social Care (Blackpool Area). Are you passionate about helping adults live independently and safely in their communities? We’re looking for a dedicated Case Assessor to join our Adult Social Care team in the Blackpool area. This is a fantastic opportunity to make a real difference while working in a supportive and professional environment.

Job Responsibility:

  • Conducting assessments and reviews to support adults in maintaining independence
  • Developing, monitoring, and reviewing personalised care packages
  • Working closely with colleagues, providers, carers, and external agencies to deliver seamless care
  • Ensuring services comply with the Care Act (2014) and Mental Capacity Act (2005)
  • Supporting and empowering individuals to make informed choices about their care
  • Advocating for vulnerable adults and ensuring safeguarding concerns are raised appropriately

Requirements:

  • Experience working in social care and inter-agency settings
  • Knowledge of adult social care, the Care Act (2014), and relevant policies
  • Excellent communication, negotiation, and IT skills
  • Ability to build strong relationships with service users and colleagues
  • Strong record-keeping, problem-solving, and decision-making skills
  • GCSEs/NVQ Level 2 (or equivalent) including Maths and English
  • Enhanced DBS with Adult Barred List check
  • Passion for empowering vulnerable adults and supporting them to live independently
