As a Care Team Manager, you’ll play a key role in leading a team of dedicated caregivers, making a real impact in the lives of our patients.
Job Responsibility:
Oversee and coordinate personal care services to ensure client plans of care are followed
Conduct start-of-care and routine client visits to assess care delivery and safety
Supervise, train, coach, and evaluate caregivers
Monitor client physical and mental status and communicate changes appropriately
Coordinate schedules and staffing to meet service needs, including planned and unplanned absences
Maintain accurate client and caregiver documentation and schedules
Review utilization, billing, payroll, and authorization reports for accuracy
Support caregiver onboarding, orientation, and ongoing training
Partner with Talent Acquisition to support caregiver recruitment and community events
Ensure compliance with state, federal, payer, and Elara Caring requirements
Foster positive relationships with clients, families, case managers, and community partners
Actively participate in branch growth and operational initiatives
Requirements:
High School Diploma or GED required
Associate’s or Bachelor’s degree preferred
Experience working with or supporting elderly and/or disabled individuals
Two (2) years of experience in a fast-paced office or healthcare setting preferred
Prior leadership, coordination, or supervisory experience preferred
Requirements vary by state and may include RN or LPN licensure, college coursework, or equivalent healthcare experience
Knowledge of Medicare/Medicaid and home care services preferred
Strong computer, communication, and organizational skills
Ability to multitask and manage priorities in a fast-paced environment
Leadership skills with the ability to motivate and support others
Professional verbal and written communication
Bilingual skills may be required based on client needs
Reliable transportation and ability to travel locally and occasionally to support audits or operational needs
Nice to have:
