Care Team Leader

• Provide professional leadership for team in support of HammondCare's Mission
• Lead team of Community Care staff providing direct care to clients
• Ensure staff understand individual client needs and preferences
• Provide services tailored to individual client needs within budget
• Lead, support and mentor team
• Manage recruitment of new Care Workers
• Ensure all working validations, education, development plans and resources are available

• Certificate IV in Aged Care (Desirable)
• HR or similar degree
• Minimum 2 years relevant industry experience
• Knowledge of Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme
• Performance coaching experience
• Management of a team in multi-site environment
• Current driver's licence and safe driving record
• Willingness to travel between clients, sites and services
• Strong organisational, administrative and computer skills
• Empathy and patience
