
Bachelor's Degree required. Social Work License, and two years of experience working with elderly or disabled adults preferred. Recent experience with elders, those who have been diagnosed with ID/D, ABI, or who have chronic conditions affecting their ability to live safely in the community. Knowledge of AFC and DDS regulations and guidelines. Strong organizational and interpersonal skills. Ability to work collaboratively with a multidisciplinary team. Attention to detail and organizational skills. Valid driver's license with an excellent RMV record (subject to RMV background check). Background check required.
