Job Description:

Field-based Care Manager RN position serving dual eligible Medicare and Medicaid populations in Southwest Michigan. Responsible for assessing, planning, implementing, and coordinating case management activities to evaluate medical needs and facilitate member wellness.

Job Responsibility:

Assess, plan, implement, and coordinate all case management activities
Develop proactive plan of care to address identified issues
Use clinical tools to evaluate member's needs and benefits
Apply clinical judgment to reduce risk factors and barriers
Conduct assessments considering information from various sources
Use holistic approach to assess need for referrals
Collaborate with supervisor and key stakeholders
Utilize case management processes in compliance with regulations
Utilize motivational interviewing skills for member engagement

Requirements:

Registered Nurse with active MI state license in good standing
Confidence working at home/independent thinker
Ability to travel within designated geographic area
Excellent analytical and problem-solving skills
Effective communications, organizational, and interpersonal skills
Ability to work independently
Effective computer skills including navigating multiple systems
Proficiency with MS Word, Excel, Outlook, and PowerPoint

Nice to have:

Care Management experience
Discharge planning experience
Home health care coordination experience
Certified Case Manager certification
BSN degree
