The Care Manager plays a crucial role in ensuring the quality of care delivered to clients and building a robust network of community contacts to enhance community services. Success in this role will be measured by the satisfaction levels of both customers and caregivers, along with the effective impact on the community through established relationships.
Job Responsibility:
Oversee quality of care delivered to clients
Develop and maintain a network of community contacts for service enhancement
Achieve high levels of customer and caregiver satisfaction
Manage approximately 70 clients effectively
Participate in 2–3 face-to-face prospect meetings weekly to support sales growth and client conversion
Convert potential clients into active clients for Careside
Collaborate with other team members to share insights and improve service delivery
Requirements:
Must be an EN or RN with relevant qualifications
Previous experience in care management or related fields preferred
Excellent organizational skills
Strong interpersonal and communication abilities
Ability to work independently and handle multiple clients
Resilience in challenging situations
Must possess a valid driver’s license for travel to meetings
