Job Description

We are currently seeking a passionate and experienced Care Manager to join our operations team. In this role, you will be responsible for delivering case management that is truly person-centered and reablement-focused.

Job Responsibility

  • Holistic Care Planning: Assess client needs to develop collaborative, strengths-based Care Plans that facilitate client ownership of their own decision-making
  • Funding & Budget Management: Manage individual client budgets effectively
  • MDT Collaboration: Facilitate referrals and work closely with internal and external stakeholders
  • Monitoring & Review: Review client Care Plans and goals at agreed intervals
  • Inclusive Service Delivery: Provide culturally appropriate care and services to CALD, ATSI, and other Special Needs clients
  • Administrative Compliance: Comply with all reporting and data requirements

Requirements

  • Relevant tertiary qualification in the community, health, or aged care sector
  • Demonstrated experience in case management within the aged, community, or disability sectors—ideally working with clients with complex needs
  • Practical understanding of the financial principles of individualized client funding and experience tracking individual budgets
  • Highly developed communication, negotiation, and interpersonal skills
  • Highly developed proficiency using computers and the Microsoft Office Suite
  • Valid Driver’s Licence and a registered, insured motor vehicle
