Care Manager - Home Care

• Manage a caseload of up to 75 clients, ensuring the highest standards of care
• Conduct weekly client visits to assess needs and monitor care plans
• Oversee the administration of home care packages, ensuring compliance and quality service delivery
• Collaborate with care partners and support staff to develop tailored care plans
• Implement best practices and policies to enhance client satisfaction and operational efficiency
• Provide support and mentorship to team members in the field

• Proven experience in case management or as a care partner within the home care industry
• Strong knowledge of home care packages and relevant regulations
• Previous aged care experience preferred
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• 2 years experience
• Cert 4 in Individual support
• Diploma in community services
• diploma of nursing or bachelors of nursing
• Vocational/Professional Qualification
