The Care Associate Behavioral Health Analyst provides services to clinical teams in support of optimal care.

Job Responsibility:

  • Assists members locate services
  • Facilitates follow-up calls
  • Collects information on member progress and adherence to treatment plan
  • Provides information to members regarding mental health and substance abuse benefits, community treatment resources, mental health managed care programs, and Aetna policies and procedures
  • Reviews member adherence with treatment plan, identifies obstacles to compliance, identifies resources to address social determinants of health and barriers to stabilization
  • Makes referrals to care connections for those without adequate aftercare appts
  • Serves as liaison between inpatient staff at hospitals to assure timely discharge appts
  • Participates in reporting activities including data collection, tracking, analysis and preparation
  • Activities may include QI initiatives
  • post discharge and recidivism follow up reporting
  • Works closely with clinicians in collaborative support

Requirements:

  • Must reside in OHIO
  • 2 years' Healthcare or Behavioral Health experience
  • Required to use a residential broadband service with internet speeds of at least 25 mbps/3mbps
  • Must possess reliable transportation and be willing and able to travel up to 10% of the time
  • High School Diploma/GED

Nice to have:

  • 2 years Behavioral Health experience working with mental health population
  • Communicating for impact, handling service challenges, maximizing work practices and organizational excellence
