The Care Associate Behavioral Health Analyst provides services to clinical teams in support of optimal care.
Job Responsibility:
Assists members locate services
Facilitates follow-up calls
Collects information on member progress and adherence to treatment plan
Provides information to members regarding mental health and substance abuse benefits, community treatment resources, mental health managed care programs, and Aetna policies and procedures
Reviews member adherence with treatment plan, identifies obstacles to compliance, identifies resources to address social determinants of health and barriers to stabilization
Makes referrals to care connections for those without adequate aftercare appts
Serves as liaison between inpatient staff at hospitals to assure timely discharge appts
Participates in reporting activities including data collection, tracking, analysis and preparation
Activities may include QI initiatives
post discharge and recidivism follow up reporting
Works closely with clinicians in collaborative support
Requirements:
Must reside in OHIO
2 years' Healthcare or Behavioral Health experience
Required to use a residential broadband service with internet speeds of at least 25 mbps/3mbps
Must possess reliable transportation and be willing and able to travel up to 10% of the time
High School Diploma/GED
Nice to have:
2 years Behavioral Health experience working with mental health population
Communicating for impact, handling service challenges, maximizing work practices and organizational excellence
