Are you a Care Home Assistant Practitioner looking to further develop your skills? Join our team as a Care Home Assistant Practitioner and help us deliver truly exceptional care to our residents. As a Care Home Assistant Practitioner, you’ll play a key role within our care team. You’ll be responsible for assessing individual needs, developing person-centred care plans, and ensuring that medical, physical, and emotional support is delivered to the highest standard. You’ll also supervise and mentor care staff, helping them grow and maintain best practices.

Job Responsibility:

  • Leading and supporting the care team to deliver high-quality care
  • Work collaboratively with Dementia Therapists, Nurses, and wider multidisciplinary teams
  • Monitoring residents’ wellbeing and updating care records
  • Liaising with healthcare professionals and families
  • Supporting residents with personal care, meals, and daily activities
  • Ensuring the home remains clean, safe, and welcoming
  • Promoting dignity, independence, and choice for every resident
  • Participating in audits, inspections, and team meetings
  • Provide support to nursing team in the implementation of nursing care
  • Administer medication (where required) in line with prescriptions and maintain accurate medication records

Requirements:

  • Has a minimum of 12 months experience working in care, ideally elderly or dementia
  • Has a completed Level 3 Diploma for Adult Care
  • Has completed a CHAPs accredited training course
  • Has a strong administration ability with a good level of IT literacy
  • Able to recognise their own limitations when working, to always ensure safe practice
  • Compassionate, patient, and reliable
  • A great communicator and team player
