The Care Coordinator is responsible for supporting members by conducting health and psychosocial assessments, developing individualized care plans, and coordinating services with an Inter-Disciplinary Care Team (ICT). This role emphasizes telephonic and electronic outreach to ensure members receive timely, culturally competent support. The Care Coordinator focuses on completing assessments and initiating care plans for a high volume of new members, identifying barriers to care, implementing interventions, educating members and caregivers about available resources and benefits, and collaborating with facility and community-based care managers to facilitate smooth transitions and prevent service duplication. The position requires accurate, timely documentation and sharing process improvement ideas to enhance workflows. The work environment is primarily office-based or remote, with standard business hours and minimal weekend work.
Job Responsibility:
Conduct health and psychosocial assessments for new members
Develop and document individualized care plans in collaboration with the Inter-Disciplinary Care Team (ICT)
Identify barriers to care and implement appropriate interventions
Educate members and caregivers on available resources and benefits
Coordinate with facility and community-based care managers to ensure smooth transitions and prevent duplication of services
Maintain accurate and timely documentation of all member interactions and care activities
Share process improvement ideas to enhance care coordination workflows
Utilize telephonic and electronic outreach to engage members and provide support
Ensure culturally competent support is provided to all members
Requirements:
Diploma, Associate’s, or Bachelor’s in healthcare-related field or equivalent experience
3+ years in care coordination, case management, discharge planning, or related healthcare role
Strong communication, organization, and documentation skills
Intermediate proficiency in MS Office
Knowledge of healthcare regulations and community resources
