Care Coordinator

• Rostering and daily management of Care Staff who are based in the community
• Working with multi-disciplinary teams and professional bodies to ensure outstanding care is delivered and to positively promote our services
• Complete quality calls with care staff and service users
• Provide reports to senior management as and when required
• Provide support to our teams as part of a rostered-on call system
• Manage referrals from commissioners and local authority
• Ensure full capacity is utilised

• Experienced Co-ordinator / scheduler doing coordination and rota management
• Vibrant, self-motivated and passionate about making a difference to the lives of vulnerable people
• Drive and Resilience
• Excellent verbal and written communication skills, comfortable on the phone and writing emails
• Experience of working in a fast paced environment and flexible
• Strong attention to detail and be a completer/finisher
• A team player with strong interpersonal skills
• Holding a full driving license and access to your own vehicle
