Job Description

We are seeking a compassionate and detail-oriented Care Coordinator to support individuals by providing information, referrals, and guidance in a fast-paced, service-driven environment. This role requires strong communication skills and the ability to remain calm and effective in high-stress situations.

Job Responsibility

  • Provide information and referral services to clients
  • Support individuals in crisis or high-stress situations with professionalism and empathy
  • Coordinate services and connect clients with appropriate resources
  • Maintain accurate documentation and case records
  • Communicate effectively with diverse populations and internal teams

Requirements

  • Bachelor’s degree in Social Work, Psychology, or related field required
  • Experience providing information and referral services required
  • Minimum 1 year of experience in crisis counseling or clinical work required
  • Experience working with veteran populations preferred
  • Strong interpersonal and communication skills
  • Ability to manage multiple tasks in a fast-paced environment
  • Proficient in Microsoft Office and general computer systems
  • Bilingual (English/Spanish)
