You’ll work closely with our Registered Manager, Clinical Team and external professionals to coordinate person-centred care that truly makes an impact.
Job Responsibility:
Coordinating care packages and staffing to meet individual needs
Creating and reviewing person-centred care plans
Acting as a key point of contact for families and professionals
Attending safeguarding and care review meetings
Ensuring services remain compliant with CQC standards
Supporting care staff with guidance and oversight
Contributing to audits and continuous service improvement
Support our network of property landlords across the UK, identify suitable accommodation and suggest adaptations to accommodate service users living there
Requirements:
Previous experience (minimum one year) in care coordination, housing management, welfare/benefits support, or a related health and social care profession would be advantageous
Commercial background / sales background
Strong organisation and communication skills
A proactive, solution-focused mindset
Ability to remain professional in challenging situations
Understanding of CQC standards
