La Causa Inc. Social Services is dedicated to empowering youth and young adults with complex mental health and developmental needs, along with their families, through compassionate, coordinated, and person-centered care. We are seeking a dedicated and collaborative Care Coordinator to join our multidisciplinary team. In this role, you will provide comprehensive care coordination for youth and young adults with high-level needs who are dually diagnosed with mental health and developmental disabilities. You will work closely with individuals and their families to develop and implement individualized plans that reflect their unique strengths, goals, and vision for a life that is happy, healthy, and safe. Through partnership, advocacy, and a recovery-oriented approach, you will help connect individuals to the resources and supports needed to achieve meaningful and lasting outcomes.
Job Responsibility:
Facilitate the development and ongoing review of individualized, strength-based service plans with youth and families
Connect individuals to mental health, developmental disability, medical, and community-based supports
Track outcomes, adjust plans as needed, and ensure services remain effective and goal-oriented
Support youth and families in navigating systems and accessing appropriate services and resources
Work closely with therapists, peer specialists, service providers, and families to ensure coordinated care
Assist in developing proactive strategies to manage and respond to crises
Maintain accurate, timely documentation in accordance with agency, state, and program requirements
Foster trusting, respectful relationships with youth, families, and team members
Fulfill all responsibilities as a mandated reporter
Participate in supervision, training, and team meetings
Requirements:
Bachelor's degree in Social Work or related field (Required)
Master's degree in Social Work or related field (Preferred)
At least one (1) year of experience working with youth with mental health and/or developmental disabilities
Experience in care coordination, case management, or community-based services preferred
Strong cultural competency and interpersonal skills
Bilingual (Spanish and English), spoken and written (Highly Desirable)
Excellent verbal, written, and documentation skills
Strong organizational and time management abilities
Proficiency with Microsoft Office Suite
Must successfully complete and pass all background checks, including an annual influenza vaccination
Valid Wisconsin driver's license, reliable transportation, and state minimum auto insurance
Strong collaboration and relationship-building skills
Critical thinking and problem-solving abilities
Ability to manage multiple priorities in a fast-paced environment
Commitment to person-centered, trauma-informed, and recovery-oriented care
Nice to have:
Master's degree in Social Work or related field
Experience in care coordination, case management, or community-based services
Bilingual (Spanish and English)
