What you’ll do? Every day as a HICSS Care Assistant is different, but your impact is always the same – helping people recover, regain independence, and remain safely at home. You will: Support individuals following hospital discharge or during periods of recovery; Deliver person-centred care in line with care plans and clinical guidance; Assist with personal care while promoting dignity and independence; Monitor wellbeing and report changes to the wider multidisciplinary team; Support mobility, medication, nutrition, and daily living activities; Work closely with healthcare professionals as part of an integrated local team

Requirements:
Previous care experience, ideally within community, reablement, or hospital discharge settings
Hold (or are willing to work towards) a relevant qualification such as a Care Certificate or NVQ/QCF Level 2 in Health & Social Care
Confident following care plans and working alongside healthcare professionals
Observant and able to recognise and report changes in condition
Reliable, committed, and able to work independently in the community
Full UK driving licence and access to own vehicle
