JourneyCare Hospice is seeking a Bridge Coordinator to join their growing hospice team in the Chicagoland area. This position works closely with our Home Health, Palliative Care, and Hospice teams to ensure a seamless transition for our patients within our care continuum.
Job Responsibility:
Identify and monitor patients within our care continuum who are at risk for needing end-of-life care.
Build strong and trusting relationships with our internal care partners to help patients receive the right level of care timely.
Provide prompt response and timely communication regarding all internal Palliative Care and Hospice inquiries.
Facilitates Goals of Care discussions with potential patients and families.
Work closely with both Home Health, Palliative Care, and Hospice teams to ensure a seamless transition within our care continuum and timely initiation of care.
Communicates with internal and community providers concerning bridge patient’s eligibility for hospice.
Participate in internal and community patient and provider education.
Assist with finding alternative services when patients are not a candidate for services.
Abides by all regulations, policies, procedures and standards.
Perform other duties as assigned.
Requirements:
Be eighteen (18) years of age or older
Associates Degree in marketing, business administration, or other related fields is preferred. Acceptable combination of experience and/or training may be considered in lieu of formal education.
Preferred- Graduate of an accredited LPN/LVN program with current license to practice in the state.
Current CPR Certification.
At least two years’ experience in a homecare, hospice and/or healthcare related field preferred.
Demonstrates good verbal and written communication, and organization skills.
Ability to complete necessary documentation appropriately, legibly, and timely.
Strong technology skills required.
Ability to handle stressful, delicate, and confidential situations.
Strong customer service skills and ability to keep good relationships with patients, families, co-workers, and other business partners.
Valid driver’s license with an automobile that is insured in accordance with state requirements.
