Job Description:

Case Management Coordinator responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate medical needs and facilitate overall wellness through integration. This role requires 75% travel for face-to-face visits with members in Miami Dade county.

Job Responsibility:

Telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members
Evaluate the medical needs of members to facilitate overall wellness
Effectively manage a caseload that includes supportive and medically complex members
Develop proactive course of action to address issues presented to enhance short and long-term outcomes
Determine appropriate services and supports due to member's health needs including Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports

Requirements:

Bi-lingual, English, and Spanish
Must reside in Miami Dade county, specifically in the surrounding Homestead area
Willing and able to travel 75% of their time to meet members face to face in Miami Dade county and surrounding areas in FL
1+ year of experience in behavioral health or a long-term care setting
Bachelor's degree required, preferably in social work or related field required

Nice to have:

Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
Effective communication skills, both verbal and written
Managed Care experience
Computer proficiency in Microsoft Word, Excel, and Outlook required
Case management and discharge planning experience
