Job Description

The Behavioral Health Patient Navigator (BHPN) position is part of the Pediatric Behavioral Health and Wellness, Integrated Behavioral Health Satellite based in Golisano Children’s Hospital Pediatric Practice (GCHPP). The BHPN ensures patients receive the best care available to them by guiding patients through all the various complexities of the healthcare system. The role supports GCHPP patients and their families through consultation regarding community resources, preparation and follow up on referrals, and participation in appointments and community-based visits when clinically indicated. The BHPN provides field-based case management services, may serve as a main point of contact for patients, builds strong relationships with patients in order to help patients stay engaged in behavioral and medical care and adhere to their care plan. The BHPN Identifies critical resources, helps navigate through health care services and systems, and promotes patient health.

Job Responsibility

  • Provide consultation to medical and mental health clinicians regarding community resources for their patients and patients’ families
  • Meet with patients and/or caregivers to assist with paperwork, correspondence, etc., necessary to ensure linkage to appropriate resources
  • as well as assist with access to urgently needed resources (e.g., food, housing, medical care)
  • Participate in evaluation and treatment appointments with mental health clinicians and patients/families as needed
  • Responsible for documenting patient and family contacts and patient progress in the electronic medical record
  • For patients in acute crisis, collect information, prepare referrals, and follow-up on referrals to necessary services in collaboration with clinicians
  • Make collateral contacts for new and existing patients and families
  • Make community-based visits when clinically indicated to support youth and family advocacy efforts (e.g., school visits, CSE meetings, DHS)
  • Provide telephone screens/ triage to families seeking GCHPP behavioral health services for their child or adolescent
  • Participate in designated service meetings

Requirements

  • Bachelor's degree in healthcare related field required
  • 1 year experience in community health, human services, and/or patient liaison/advocate roles preferred
  • Lived experience working with the mental health system related to a family member preferred
  • Must possess a valid New York State Driver's License, satisfactory driving record, and access to a reliable automobile required
  • BCPA-Board Certified Patient Advocate upon hire preferred
