As a Bank Registered Nurse Band 5, you’ll be part of our valued team in our Swindon Hospital at Home service. You will be part of the Clinical Team, working across organisational boundaries to bring together the expertise of acute services with community services. We support both step-up patients with a primary focus on admission avoidance and step-down patients supporting early discharge from the acute setting. You will be working as a Nurse, joining innovative and forward-thinking colleagues who are compassionate and committed to the people in the local community who use our services and go above and beyond to ensure they receive the best quality care. You will work with a good degree of independence, carrying out tasks and responsibilities delegated by the lead clinician. You will be an important part of the team, helping to meet the needs of patients in community settings, initially focusing on those referred through the 2-hour Urgent Community Response (UCR), and later supporting the development of broader referral pathways. Working within a multidisciplinary team and across primary, secondary, acute, and social care services in Wiltshire, you will play a key role in coordinating specialist interventions for people who might otherwise require hospital care, or who need timely support to leave emergency departments safely. You will take responsibility for your clinical caseload and help ensure smooth coordination of care throughout each patient’s journey.
Job Responsibility:
Clinical practice – Providing evidence-based advanced clinical skills to support appropriate patient care, under the guidance of a registered clinician. This includes assessing patients, interpreting tests within your scope of practice, and planning care and treatment
Recognise early signs of deterioration, respond promptly, escalate when needed, and document your actions clearly
Management – Organising your time and workload effectively, advocating for patients, managing your caseload day to day, offering advice and treatment recommendations, working closely with clinicians in the Virtual Ward team, and contributing to supervision
Leadership – Serving as a positive role model for colleagues and promoting collaborative working
Help challenge traditional boundaries in a supportive way while upholding high standards of quality, safety, and professional practice
Supporting the learning environment – Demonstrating commitment to your own professional development and supporting the learning of colleagues and student nurses

Requirements:
Registered nurse
Evidence of study at Level 5 or above
Advanced clinical assessment skills
IV therapy competency
Able to work autonomously and as part of a team
Experienced in the use of NEWS2 diagnostic tool and when to escalate
Contribute to the development of others, supervising and teaching, mentoring, including student Nurses
Previous experience in elderly care, frailty, community services or relevant acute care speciality
Excellent written and verbal communication, and interpersonal skills
Ability to adapt to the demands of a constantly changing environment
Good IT skills
Work within the NMC code of conduct
Knowledge of community environment
Good Communication skills including IT literacy and knowledge of computer systems
Undertake evidence based clinical practice
Experience of caring for people with long term conditions
Experience of pressure ulcer prevention
Awareness of self-neglect, safeguarding and abusive situations to be the patients advocate
Ability to be an effective team member. Awareness of team dynamics
Work within the boundaries of legislation, policies, procedures, and standards
clean driving licence, Car driver and access to a vehicle for daily use
Nice to have:
Knowledge of clinical systems such as Systmone
Independent prescribing qualification
Postgraduate certificate in advanced practice or relevant specialty
