Job Responsibility

Ensures the timely and adequate delivery of hospice services to the terminally ill patient and their family, operating within the plan of care as established by the hospice team and attending physician
Assesses the total needs of the patient/family during regularly scheduled and after hour home visits
Documents: assessment, identified problems, nursing interventions, goals, and outcomes of interventions
Coordinates total patient/family hospice care under the supervision of the Director and with the interdisciplinary team, the attending physician, and other providers

Collaborates with the interdisciplinary team in the development, review and revision of the clinical component of patient/family plan of care, including exchange of information, review of problems, assessing effectiveness of interventions and documenting outcomes
Facilitates the delivery of hospice services to patients residing in long term care facilities, documenting joint coordination of care with facility professional staff, and attending facility care plan meetings as appropriate
Participates in on-call rotation for delivery of care after office hours, on weekends, and holidays

Job Responsibility

Provides and/or fabricates assistive devices and/or splints to enhance patient's functional independence
Monitors and evaluates outcomes and appropriately adjusts individualized treatment plan based on patient's response, and provides status updates to physicians
Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs
Plans for and completes patient discharge including providing written home programs, patient/family teaching, referral for community services, and ordering equipment
Completes all required documentation including patient progress notes, team conference reports, discharge summaries, and records patient care and progress as well as daily statistics, billing, and scheduling
Applies insurance guidelines regarding authorization and number of visits
Responsible for cleaning up after patient treatment, putting away equipment, and keeping area neat and orderly
