Join our team as a Discharge to Assess Physiotherapist and play a key role in supporting patients’ safe transition from hospital to home or care placements.
Job Responsibility:
Undertake assessment and intervention in the community and care homes under the D2A pathways
Assess patients’ therapy and care needs to promote safety and independence
Provide interventions aimed at reducing risk of re-admission or harm, supporting patients onto the best pathway for their needs
Coordinate services by completing person-centred assessments, developing and implementing care and therapy plans
Assess equipment needs, support discharge planning, and set rehabilitation goals to optimise function and independence
Work autonomously within professional guidelines and the service’s policies and procedures
Assess for and arrange health or social care packages to meet patients’ needs (training provided as needed)
Requirements:
Current and valid HCPC registration
Previous experience in community rehab, discharge to assess, or care home settings
Strong communication and interpersonal skills
Ability to work independently and within a multidisciplinary team
Commitment to delivering high-quality, person-centred care
Full UK driving licence and access to a vehicle for community visits
