Job Description:

Autism Case Manager for Adult Programs. ServiceNet, a leading Human Services agency, is seeking compassionate, dedicated, and enthusiastic individuals to join our team. As a Case Manager / Autism Specialist, you will work directly with a caseload of adults in their own residences and in local communities with them. Support is focused on goals identified by the individual and their team including household management, executive functioning, social/recreational, health care management, and other areas of life. Adults programs work with a caseload of individuals who, with the right supports, can remain living independently.

Job Responsibility:

  • Be part of teams that provide outreach support to adults with autism
  • Partner with them in creating and reaching their goals, increasing their skills, and making connections within their local communities
  • Support goals such as living more independently, health care coordination, grocery shopping, meal preparation, care of living space, financial decision making, crisis support, accessing and connecting with the local community, recreation and making friends, day to day problem solving

Requirements:

  • Bachelor’s degree preferred or education equivalent to a 4-year college degree
  • Human Services residential outreach experience
  • Knowledge of Autism services and treatments
  • Over 1 year supporting adults with autism
  • Reliable vehicle, valid driver's license and willing to transport individuals when needed

  • Maintain confidentiality according to agency guidelines, follows ethical guidelines
  • Maintain knowledge of community resources that can be utilized to support participant needs (i.e., transportation, medical, childcare, utilities, etc.). Make referrals, advocate, and follow-up
  • Completes training requirements on an annual basis to demonstrate program-specific knowledge
