By utilizing your nursing skills and knowledge, you can impact the lives of clients, their families and the caregivers who provide hands-on services.

Job Responsibility:

Assess clients to create individualized care plans
Audit client and caregiver records
Educate caregivers
Help clients age in place safely
Build and lead a team of caregivers
Provide wound care, IV infusions, tracheostomy management, feeding tube management, home ventilator management, transitional care
Work with client's medical team, family, and caregiver
Prevent re-hospitalization

Requirements:

Registered Nurse
Nursing skills
Clinical judgment
Expertise in creating individualized care plans
Experience with wound care, IV infusions, tracheostomy management, feeding tube management, home ventilator management, transitional care
