Join our Community Supportive and Palliative Care Team as an Assistant Practitioner (Band 4, 30 hours / 0.8 WTE). An exciting opportunity has arisen for an Assistant Practitioner to work within the well-established Community Supportive and Palliative Team. The successful candidate will work flexibly over 4 days Monday Friday 8.30 16.30 (excluding bank holidays). The service is based at Radford Health Centre and provides specialist support to palliative patients and their families across a variety of settings across Nottingham City including the patients own home. The successful candidate will work within the team to support both the Nursing and Rehabilitation staff already in post. The role will help to support and improve the delivery and provision of palliative end of life care in the community for all palliative care patients with a life limiting diagnosis, in order to improve quality of life, promote independence and improve the patients and carers experiences at the end of life.

Plan, prepare and carry out rehabilitation and/or nursing tasks
Evaluate progress
Liaise with multi-professional teams
Maintain accurate patient records
Undertake administrative and clerical duties
Take referrals into the service
Provide support to the Triage nurse
Assist in induction, training and education of other staff
Be involved in appraisal systems and clinical supervision
Participate in In-Service training
Comply with mandatory and service specific training
Educate clients and carers in safe use of equipment

Requirements
Foundation degree or NVQ 3 / equivalent or demonstrable experience
Experience working within a multi-disciplinary team
Experience managing own workload
Experience maintaining accurate records and data recording
Experience working with people with palliative or long-term conditions
Understanding of professional area procedures and practices
Knowledge of health and social care provision
Ability to identify need and risk in community healthcare
Knowledge of assessment and care planning
Understanding and experience of rehabilitation and/or nursing care processes
Ability to promote independence and self-care
Awareness or use of daily living equipment and adaptations
Ability to assess patients needs and take appropriate action
Person-centred approach
