Job Description:

Pivotal leadership role shaping the future of hospital discharge pathways and driving meaningful change across the county. Key member of the Adult Social Care (ASC) Senior Leadership Team.

Job Responsibility:

Provide strategic and operational support to the Head of Service
Take a lead role in the management and development of the Hospital Discharge and Assessment Team
Drive service improvement
Ensure value for money
Deliver high-quality customer service

Requirements:

Qualified social worker registered with Social Work England
Proven experience in leadership, management, and development of teams
Substantial experience working within adult social care
Experience working with partner agencies, stakeholders, and other professionals at a senior level
Confident and resilient
Ability to work in a fast-paced environment making rapid but considered decisions
In-depth knowledge and understanding of the legal and policy framework in relation to adult social care
