Job Description:

This Analyst Case Management position is with Aetna’s Long-Term Services and Supports (LTSS) team and is a field-based position. The role involves evaluating member needs through care management tools, coordinating care plans, and promoting lifestyle changes to achieve health outcomes. Travel up to 50-75% of the time is required to meet members face-to-face.

Job Responsibility:

Conducts comprehensive evaluation of referred member's needs and recommends an approach to case resolution
Coordinates and implements assigned care plan activities and monitors care plan progress
Consults with case managers, supervisors, Medical Directors and other health programs to overcome barriers
Identifies and escalates quality of care issues
Utilizes influencing skills to ensure maximum member engagement
Provides coaching, information and support for ongoing medical or healthy lifestyle choices
Utilizes case management and quality management processes in compliance with guidelines.

Requirements:

Candidate must reside in the North Side of Chicago/Skokie, IL or surrounding areas (Applicable Zip Codes: 60613, 60614, 60618, 60625, 60626, 60630, 60640, 60645, 60646, 60657, 60659, 60660, 60202, 60076)
Must possess reliable transportation and be willing and able to travel up to 50-75% of the time
Mileage is reimbursed per our company expense reimbursement policy
Candidate must have a valid IL Driver's License
2+ years of experience in behavioral health, social services or appropriate related field equivalent to program focus.

Nice to have:

Case management and discharge planning experience
Managed care experience
Microsoft Office (Excel) experience.
