Ambulatory Clinical Pharmacist - Immunology (Pulmonology/Rheumatology)

The Ambulatory Clinical Pharmacist provides comprehensive pharmaceutical care for patients in the ambulatory setting. This consists of ensuring drug therapy that results in safe, effective, and cost-conscious use of pharmaceuticals. In addition, the pharmacist ensures patients have appropriate access to prescribed medications and are provided with medication education as well as adherence tools when needed/appropriate. This pharmacist will work in the pulmonary and rheumatology clinics alongside the multidisciplinary team to serve patients with pulmonary (interstitial lung diseases, eosinophilic disorders / vasculitis, non-cystic fibrosis bronchiectasis) and various rheumatologic disease states on complex medication(s). They will be responsible for initial clinical evaluation, patient education and ongoing monitoring of all patients with pulmonary / rheumatologic diseases on specialty medications.
