Ambulatory Clinical Pharmacist - Cardiology

The Ambulatory Clinical Pharmacist provides comprehensive pharmaceutical care for patients in the ambulatory setting. This consists of ensuring drug therapy that results in safe, effective, and cost-conscious use of pharmaceuticals. In addition, the pharmacist ensures patients have appropriate access to prescribed medications and are provided with medication education as well as adherence tools when needed/appropriate. This pharmacist will work in the cardiology clinics to serve patients with various disease dates. This position will be responsible for conducting referral-based pharmacist-led comprehensive medication management appointments for patients with Guideline Directed Medical Therapy (GDMT) for Heart Failure, Lipid Management and GLP-1 Initiation and Titration. In addition, this position will provide initial clinical evaluation, patient education and ongoing monitoring of all patients with cardiac diseases on specialty medications (Cardiac Amyloidosis, Obstructive Hypertrophic Cardiomyopathy).
