Job Responsibility

Answer emergent calls from patients and families
Provide after-hours visits to address immediate concerns or changes in condition
Deliver skilled nursing care based on patient goals and the physician's plan of care
Assess and evaluate patient status
give guidance and oversight when LPN support is involved
Develop and update care plans based on nursing assessments
Administer medications and treatments as prescribed
Support preventive, restorative, and rehabilitative care needs
Educate patients and families on symptom management and care techniques
Offer support and reassurance during challenging and emotional moments
