We're seeking a compassionate Licensed Practical Nurse (LPN) to join our hospice After Hours team. As an LPN, you'll provide direct patient care under the supervision of a Registered Nurse, following individualized care plans and ensuring every patient receives the highest standard of comfort and support. You'll collaborate closely with an interdisciplinary team to deliver care that meets physical, emotional, and spiritual needs.
Job Responsibility
Provide direct patient care as outlined in the plan of care established by the Registered Nurse
Implement current nursing practices based on comprehensive assessments and individualized care plans
Document care accurately and in a timely manner in the EMR—right at the bedside—to reflect the plan of care
Educate and support patients, families, and caregivers regarding diagnoses and self-care plans as directed by the RN or provider
Manage medical equipment and supplies effectively and efficiently to ensure seamless patient care
Provide appropriate pain and symptom management, evaluate patient responses to treatments and medications, and collaborate closely with the RN and provider regarding changes or concerns
Facilitate cohesive, team-based patient care in alignment with organizational quality initiatives, policies, and protocols
Participate in team meetings, interdisciplinary collaboration, quality improvement initiatives, and ongoing professional growth and development
Maintain reliable transportation, including a valid driver's license and insured vehicle, and be willing to travel throughout the full service area
Perform additional duties as assigned by the manager or team leader to support exceptional patient care
Requirements
Graduate of an accredited practical or vocational nursing program
Current LPN license in Virginia
CPR certification and at least two years of nursing experience (community health, hospice, or medical-surgical preferred)
Excellent communication, organization, and critical thinking skills
Reliable transportation, valid driver's license, and automobile insurance
A compassionate, self-directed approach and a commitment to serving your community
Continuously stand and walk throughout the workday, with frequent squatting, stair climbing, bending, kneeling, twisting, reaching overhead, and sitting as needed
Safely lift, reposition, and assist patients—including lifting up to 50 pounds—and remain in physically demanding or uncomfortable positions for extended periods of time
Maintain visual acuity sufficient to observe and assess changes in patient condition
Demonstrate hearing acuity adequate to auscultate blood pressure, understand verbal instructions and patient communication, respond to emergency alarms, and answer telephone calls promptly and clearly
Apply sustained attention, critical thinking, and sound clinical judgment to safely perform all essential job functions
