Job Description:

Job Responsibility:

  • Provide direct patient care as outlined in the plan of care established by the Registered Nurse
  • Implement current nursing practices based on comprehensive assessments and individualized care plans
  • Document care accurately and in a timely manner in the EMR—right at the bedside—to reflect the plan of care
  • Educate and support patients, families, and caregivers regarding diagnoses and self-care plans as directed by the RN or provider
  • Manage medical equipment and supplies effectively and efficiently to ensure seamless patient care
  • Provide appropriate pain and symptom management, evaluate patient responses to treatments and medications, and collaborate closely with the RN and provider regarding changes or concerns
  • Facilitate cohesive, team-based patient care in alignment with organizational quality initiatives, policies, and protocols
  • Participate in team meetings, interdisciplinary collaboration, quality improvement initiatives, and ongoing professional growth and development
  • Maintain reliable transportation, including a valid driver’s license and insured vehicle, and be willing to travel throughout the full service area
  • Perform additional duties as assigned by the manager or team leader to support exceptional patient care

Requirements:

  • Graduate of an accredited practical or vocational nursing program
  • Current LPN license in Virginia
  • CPR certification and at least two years of nursing experience (community health, hospice, or medical-surgical preferred)
  • Excellent communication, organization, and critical thinking skills
  • Reliable transportation, valid driver’s license, and automobile insurance
  • A compassionate, self-directed approach and a commitment to serving your community
