As an ANP, you will use your expert palliative care skills and highly experienced clinical judgement to contribute to the excellent evidence-based specialist treatment and care the charity provides to patients with complex life-limiting illnesses. You will be responsible for assessing and diagnosing patients and planning and implementing their care to ensure optimum outcomes. You will also assist the Community Cluster Team Leader with line management responsibilities and the daily clinical leadership of junior colleagues within the community team. The role involves travel within the community setting, covering the Mid Devon area predominantly, and participating in the seven-day service provision to include on-call.
Job Responsibility
Manage a caseload of patients
Provide holistic care, expert advice, and intervention
Collaborate effectively with local GPs and Community Nurses to deliver an excellent Hospiscare service
Assist the Community Cluster Team Leader with line management responsibilities
