Job Description:

An experienced Advanced Nurse Practitioner is required to deliver autonomous, patient-centred care in a primary care setting. The role involves managing acute presentations via face-to-face, telephone, video consultations, and occasional home visits.

Job Responsibility:

  • Full patient management: assessment, diagnosis, treatment, and evaluation
  • Clinical decision-making and safe management of acute cases
  • Working alongside GPs to manage daily demand
  • Collaborative team working and contribution to service delivery
  • Supporting policies, procedures, and potential leadership in clinical areas

Requirements:

  • MSc in Advanced Practice (or equivalent)
  • Post-registration experience
  • Experience in community care and minor illness management

Nice to have:

  • GP/primary care experience
  • Chronic disease and palliative care experience
  • Nurse-led triage and minor illness/injury management
  • Additional qualifications (e.g. diabetes, respiratory, cytology)
