Working as part of a multidisciplinary team you will be trusted to make complex clinical decisions, manage undifferentiated presentations, and influence how services are designed and delivered across the system. Job Responsibility:
Practise as an independent and autonomous clinician, managing patients with undifferentiated, acute and chronic conditions across face-to-face, home visiting, remote and digital consultation models
Undertake comprehensive clinical assessment, diagnosis and management, demonstrating advanced clinical reasoning, risk assessment and safety netting
Initiate, interpret and act on appropriate investigations and referrals in line with local and national pathways
Prescribe and manage medications safely and effectively in accordance with national guidance, local formularies and EMIS decision-support tools
Contribute to flow, continuity and demand management across integrated services, including prevention of unnecessary hospital admissions
Respond confidently to complex, urgent and emergency presentations, initiating immediate care as required
Safeguard children, young people and adults, recognising risk, acting within statutory frameworks and escalating concerns appropriately
Work collaboratively with GPs, nurses, paramedics, pharmacists, operational teams and external partners to deliver seamless, patient-centred care
Support system-wide working by ensuring patients receive the right care, in the right place, at the right time
Contribute to the development of new models of care that enhance access, quality and patient experience
Act as a senior clinical decision-maker within the multidisciplinary team
Provide visible clinical leadership, supporting safe practice, innovation and continuous improvement
Contribute to service redesign, pathway development and quality assurance activity
Support the investigation and learning from clinical incidents, audits and significant events
Promote a positive learning culture, role-modelling professionalism and reflective practice
Maintain and develop advanced clinical and prescribing competence in line with the four pillars of ACP practice
Support the education, supervision and development of colleagues, including trainees and students
Contribute to teaching, mentorship and clinical assessment as appropriate
Apply principles of evidence-based practice, audit and critical appraisal to improve care quality and outcomes
Practise in accordance with professional codes, national standards and Taurus Healthcare policies
Maintain accurate, timely and high-quality clinical records using approved clinical systems
Uphold information governance, confidentiality and data protection requirements always
Take responsibility for personal accountability, recognising scope of practice and seeking support when required
Participate in clinical supervision, appraisal and personal development planning
Maintain mandatory training and professional registration
Demonstrate flexibility to support service delivery across evenings, weekends and bank holidays as required

Requirements:
Experience of working in a multi-professional environment
Experience as an Independent Prescriber
Experience of working under pressure
Experience of telephone (remote) triage
Accurate record keeping skills
Awareness of own accountability and other aspects of NMC code of conduct for nurses in this role
An understanding, acceptance, and adherence to the need for strict confidentiality and working knowledge of Information
Works a flexible pattern of evenings weekends and bank holidays and covers team annual leave and sickness when applicable
Organised, friendly, approachable, polite, compassionate, and empathetic

Driver with clean licence
RN current NMC registration or Paramedic Health and Care Professions Council (HCPC) registration
A demonstrable commitment to professional development
Advanced Clinical Practice (ACP) qualification MSc level or equivalent (demonstrated by professional credentialling or professional equivalent)
Registered Non-Medical Independent Prescriber
Safeguarding adult and children Level 3
In date CPR training (within 12 months)
Minimum 5 years experience post qualification
Experience in managing the care of patients with undifferentiated undiagnosed conditions
Works autonomously and collaboratively, recognising when referrals are required
Practical experience of completing examination and assessments, care planning, implementing interventions, including secondary care referrals, and evaluation of outcomes
Proven ability to recognise and manage risk
Excellent communication skills both written and verbal
IT skills computer and keyboard literate
Good time management and the ability to work under pressure prioritising workload
Relay clinical information and maintain accurate, comprehensive consultation records in line with NMC requirements
Nice to have:
Experience of working in Primary Care in autonomous ACP role
Experience of EMIS clinical system and using templates
Understanding of Quality Outcomes Framework
Minimum 2 years experience in general practice
