Job Description:

Agewell is seeking a Nurse Practitioner to join our growing practice in Jacksonville, FL. The selected Advanced Practitioner Provider would serve as a link between inpatient and outpatient care as a member of the Baptist Health team.

Job Responsibility:

  • Serve as a link between inpatient and outpatient care as a member of the Baptist Health team
  • Consult with patients about their current conditions
  • Order diagnostics
  • Analyze results
  • Diagnose conditions
  • Prescribe treatment strategies, including prescription medications
  • May also actively participate in research in the field and advocate for policies that support the needs of elderly patients

Requirements:

  • Master's Degree - Nursing, required
  • Certified Nurse Practitioner
  • Licensed ARNP
  • 2+ years ARNP experience
  • 1-2 years Family Practice Experience
  • Certified Physician Assistant (PA-C) Required
  • Licensed Physician Assistant Required
