Job Description

IES is seeking an Advanced Practice Provider for our Transitions of Care Clinic located at Baylor Scott & White Medical Center – McKinney. This position will involve scheduled visits with patients discharged from the BSW McKinney emergency department. TOC Clinic operates Monday, Wednesday & Thursday. Clinic hours: 8a-4p. Maximum patient visits per day is 12. These will be high quality visits with patients recently discharged from the ED that may be vulnerable to a gap in care. These are patients with complex medical & social needs. This is a unique, and highly rewarding, opportunity to provide care to a patient population that lacks the resources to get care outside of the ED. Interrupt the cycle of repeat ED visits due to a lack of follow-up care after being discharged. The clinic is located on the campus of Baylor Scott & White - McKinney. Baylor or an Integrative Health representative will schedule patient visits. No walk-in patients.

Job Responsibility

  • Scheduled visits with patients discharged from the BSW McKinney emergency department
  • Coordinate next steps in care plan
  • Medications refills (if needed)
  • Help cut down repeat ED visits

Requirements

  • Board Certified Physician Assistant or Nurse Practitioner
  • License to practice in the state of Texas

Nice to have

APP's with a background in emergency medicine, hospital medicine or ICU experience
