Job Responsibility:
Conducting comprehensive strengths-based assessments for adults with a diverse range of needs, including older people, individuals with physical disabilities, learning disabilities, mental health conditions, and complex health needs
Developing, implementing, and reviewing creative and personalised care and support plans in collaboration with individuals, their families, and carers, promoting choice, control, and positive outcomes
Facilitating access to appropriate resources and services, including advocating for individuals to ensure their needs are met effectively and efficiently
Working collaboratively within a multi-disciplinary framework, liaising closely with health professionals, housing, voluntary sector organisations, and other council departments to ensure integrated and holistic support
Leading on complex safeguarding adult enquiries under Section 42 of the Care Act 2014, undertaking investigations, chairing safeguarding meetings, and ensuring the safety and well-being of vulnerable adults
Undertaking Mental Capacity Act (MCA) assessments and acting as a Best Interests Assessor (BIA) where qualified, ensuring statutory duties are met and individuals' rights are protected
Managing a complex and varied caseload, prioritising effectively to meet statutory timescales and service demands
Preparing clear, analytical, and defensible reports and documentation for various purposes, including funding panels, court proceedings (if applicable), and case reviews
Providing expert advice and guidance to individuals, families, and other professionals on adult social care legislation, policy, and available services
Contributing to the continuous improvement of service delivery and promoting best practice within the team and wider department
Requirements:
Qualified Social Worker with a degree in Social Work or equivalent
Social Work England registration is essential and must be current
An enhanced DBS check will be required for this role
Significant post-qualifying experience in adult social care is highly desirable
