Job Responsibility
Driving excellence in Support planning, reviews, and community integration for individuals subject to Section 117 aftercare
leading on Section 117 statutory responsibilities, balancing legal frameworks with creative, strengths-based social work
acting as a key point of expertise, shaping integrated procedures and providing guidance to the wider team
delivering a comprehensive social care service to working-age community adults experiencing moderate to severe mental health difficulties
promoting a needs-based, partnership-driven approach to service delivery alongside multi-disciplinary health teams
developing meaningful engagement with individuals and families who are traditionally viewed as difficult to engage
providing a comprehensive social care service to young people transitioning from children's services into adult provisions
acting as a specialist bridge between frameworks, requiring extensive legal literacy regarding both adult and children's statutory systems
Requirements
Professional Social Work Qualification (Degree or DipSW)
Social Work England registration
completed ASYE
Minimum of 2 years post-qualification experience
specific, substantial experience in mental health and transitions
Full current UK driving licence
a car available for daily community duties
Nice to have
Approved Mental Health Professional (AMHP) qualification
