Job Description:

Doncaster Council is seeking a Transitions Social Worker to join their dynamic Learning Disabilities team. This is a rewarding locum opportunity offering £32 per hour, working with young people aged 14-25 as they move from children's to adult services.

Job Responsibility:

  • Support young people with learning disabilities through the transition from children's to adult social care services
  • Undertake assessments, reviews, and care planning in line with the Care Act 2014
  • Collaborate with families, carers, education providers, health professionals, and multi-agency teams
  • Promote independence, choice, and well-being through person-centred planning
  • Maintain accurate records and ensure compliance with safeguarding procedures

Requirements:

  • Social Work England registration
  • Proven experience in transitions or learning disabilities social work
  • Strong knowledge of relevant legislation including the Care Act, Mental Capacity Act, and Children and Families Act
  • Enhanced DBS (on the update service preferred)
  • Ability to hit the ground running in a fast-paced environment

  • Conduct regular supervision with team members to offer feedback, set goals, and identify areas for development
  • Identify training needs and facilitate relevant training sessions or workshops for staff
  • Support staff in obtaining necessary qualifications and certifications
  • Ensure the rights, dignity, health, and privacy of service users are maintained at all times
  • Assist the people we support with daily routines, support needs, and activities, documenting progress towards personal goals
