Job Description:

Part-time Social Worker role (27.5 hours) within the Cambridgeshire Learning Disability Partnership (LDP), an integrated team of health and social care practitioners. The role involves working to a hybrid model, from the office up to three days a week, following an induction. The council's 'Transforming Lives' approach focuses on proactive, preventative, and personalised social work.

Job Responsibility:

  • Undertaking Social Care and Mental Capacity Act assessments
  • Completing Safeguarding as well as care and support planning
  • Completing Community deprivation of liberty applications to the Court of Protection
  • Provide evidence based, proportionate recording with a keen focus on strength-based practice
  • Work in partnership with adults and their informal carers to establish needs and ensure that they are met

Requirements:

  • Experienced qualified Social Worker with a Social Work England registration
  • Can work from a variety of locations and travel countywide independently
  • Excellent written and verbal communication skills
  • Ability to work both independently and as part of a multi-disciplinary team
